Worksheet with forms

As an educator you will find that using forms in questionnaires, worksheets and quizzes will help you save time when creating these documents. Forms are only useful if the documents are going to be completed using the computer. You will use Microsoft Word to create these forms-based documents.

2 Open the forms toolbar: You will need to add the forms toolbar if it is not already visible.

3 Inserting form fields for text: This is typically used for names, grades etc. How to add a text form field

6 Inserting the question: Insert the required questions and choose if you what to use a dropdown, checkbox or text form field.
