Create a worksheet with form (Intermediate level)

As an educator you will find that using forms in questionnaires, worksheets and quizzes will help you save time when creating these documents. Forms are only useful if the documents are going to be completed using the computer. You will use Microsoft Word to create these forms-based documents.

By the end of this task you could achieve the following outcomes:

Download an example - Type a worksheet, questionnaire, quizz
- Create form objects in a document like checkboxes and drop down menus
- Protect your form
- Conduct a spell check
- Save the document

Create your own forms-based document - one which you will find useful. Alternatively, follow this example. To do so you would typically work through the following steps:

1. Start Microsoft Word. See how »
2. Open the forms toolbar: You will need to add the forms toolbar if it is not already visible.
3. Inserting form fields for text: This is typically used for names, grades etc. See how »
4. Inserting the check box form field e.g. gender: male, female. See how »
5. Insert a dropdown menu bookmark e.g. Age and Subjects /Learning Area. See how »
6. Inserting the question: Insert the required questions and choose if you what to use a dropdown, checkbox or text form field. See how »
7. Protecting your form: Before you make your form available to users you must protect your form. See how »
8. Spell check your document: Conduct a spell check before saving and printing a document. See how »
